The Transition Toolkit is Ready!

After almost a year of feverish work, the Transition Toolkit is ready for distribution! TheTransition Toolkit was created for young people with disabilities and their parents, familiy members and others who support their efforts to achieve their full potential in life.

Picture of Transition ToolkitTransition is a constant life process of moving on from one phase to the next. We are constantly in transition. Sometimes transition periods are smooth. Many times, however, transition produces more anxiety, more fears and more confusion than is typical for a young person with a disability. The Transition Tool Kit is for parents and students with disabilities who are looking for a way to handle those stressful transition times better.

Our goal in creating the tool kit is to provide a functional tool that is versatile, portable, and useful.

This tool kit does several things:
1. Provides helpful educational information and strategies that work
2. A place to keep records of education plans, evaluations and other important documents
3. Helps you to organize information about your son or daughter
4. A place to keep information about your child that describes who they are.

“I wish I would have known that when….” is a common line for many parents. For parents of children with disabilities, the challenge is even more complex and sometimes overwhelming. So, parents and young people who have ‘been there and done that’ have written stories and helpful information that shares life experiences and what works and what didn’t work.
